Insurance Policy

At this time, we do not take any form of insurance.  We do, however, supply you with a Claim Form that you can submit to your insurance company for reimbursement.  Many of our patients with PPO plans get anywhere from 60% to 80% reimbursed, depending on their individual coverage.

Cancellation Policy

PLEASE NOTE: Because of the high demand on Dr. Bernhoft’s patient schedule and in fairness to others who are trying to schedule appointments, new patients are given 72 hours to cancel their initial visit. If they do not cancel their appointment before this deadline, they will be charged for the full cost of their appointment. As a courtesy, an automated reminder will either be emailed, called or texted to the contact number given when the appointment is made, but ultimately it is the patient’s responsibility to remember appointment dates and times.

All patients, whether new or existing, will have 72 hours to cancel all appointments, including treatments.

If patients do not call before the 72 hour deadline to reschedule or cancel appointments, they will be charged the full amount for their appointment.  That means if you have an appointment on Monday and you need to cancel it, you must call by Friday morning or you will be charged the full amount of that scheduled visit.
